Sample Feedback Form
Tell us what you think of your AMR Labs sample. Include the formula number and specific feedback on its texture, fragrance, appearance, and performance, along with any changes you would like our R&D team to make. Don’t feel like you have to hold back on your notes. We want you to be happy with your product, so give us the most complete feedback you can. If the sample meets your expectations, mark it as approved so we can begin the quoting process.
Please fill out this form as one per formula, even if the feedback is the same across all formulas. For example, do not group products like “Shampoo and Conditioner”. The form is meant to be per product and per formula number.
If your sample is approved, simply fill out the required fields and submit the form. Our offices will be in touch with you regarding the next steps.
If your sample is not approved, please be sure to provide as much detail regarding why. By providing as much detail as possible to our research and development team, they will be able create a sample to the utmost standard.
